Ministry purpose: To provide case management services including the development of an action plan that helps clients transitioning out of the MODCO   Housing Program. Depending on need, services may include information and referrals to resources outside of MODCO’s scope of services.

Hours: Monday—Friday / 9:00 am—5:00 pm
